+## THE PROBLEM
+
+Things are not being documented properly. Example:
+
+**Ghost footer social links** - Reddit, Bluesky, Mastodon, Kick were supposed to be added in a previous session but were NOT.
+
+This indicates a systemic documentation failure in the Chronicler handoff process.
+

+## WHAT MUST HAPPEN NEXT SESSION
+
+**BEFORE doing ANY other work:**
+
+1. **Review documentation processes**
+   - How are tasks being tracked?
+   - How are completions being verified?
+   - What's falling through the cracks?
+
+2. **Audit recent work**
+   - What else was "supposed to be done" but wasn't?
+   - Which tasks are marked complete but actually incomplete?
+   - What decisions were made but not documented?
+
+3. **Fix the process**
+   - Update FFG-STD-002 (Task Documentation) if needed
+   - Update SESSION-HANDOFF-NEXT.md template
+   - Create verification checklist for session ends
+
