fix: Correct double fastmcp typo in MCP_SETUP.md

Browse Source 
Fixed 41 instances of 'server_fastmcp_fastmcp' to 'server_fastmcp'.
This was a typo in the documentation that would prevent the MCP server
from starting correctly.

All other files in the PR correctly use 'server_fastmcp'.
