docs: Add comprehensive QA audit documentation (v2.11.0)
Added two comprehensive QA reports documenting in-depth system audit:
1. QA_EXECUTIVE_SUMMARY.md (production readiness report)
- Bottom line: APPROVED FOR RELEASE (9.5/10 quality)
- Test results: 232 tests, 100% pass rate
- Issues: 5 non-blocking deprecation warnings
- Clear recommendations and action items
2. COMPREHENSIVE_QA_REPORT.md (detailed technical audit)
- Full subsystem analysis
- Code quality metrics (9.5/10 average)
- Issue tracking with severity levels
- Test coverage statistics
- Performance characteristics
- Deprecation warnings documentation
