New skill: repomix-safe-mixer

- Scan → Report → Pack workflow with automatic blocking
- Standalone security scanner for pre-commit hooks
- Environment variable replacement guidance
- JSON output for CI/CD integration

Also updates:
- skill-creator: Simplified path resolution best practices
