Fix privacy violation: replace real names with handles on cancellation-refund page

The Trinity Appeal Process section contained real first names (Michael, Meg,
and Holly) on a public-facing document. Replaced with gaming handles
(Frostystyle, Gingerfury, unicorn20089) per Firefrost privacy policy.

Caught by Holly (The Catalyst). Thank you.
